Spectacular failure

Earlier today I was testing the failure mode of some spare parts when subjected to multiple 22J 250v blasts, and got nothing particularly interesting. A couple minutes ago I was just running a couple of fans through a bridge, and it catches on fire! It as originally a black disk about the size of a skittle, but flat, and was probably rated for 50v 1a. It was just running at 14v, and .25 amp, but all of the sudden it starts smoking like crazy and catches on fire, desoldered a lead, fusing the other ones together, and burning out a fan. I cut the power to it, and blew out the flame, and it was reduced to a distorted smoking molten blob on my desk. It only had 22awg leads going back to a beefy transformer, so the leads bubbled and smoked along their whole length. What could bring about such a failure with no warning?

Was this device part of the fan motor assembly? Where these brushless motor type fans? Was there anything left to take a picture of?

Just the fan from a pc power suply, the bridge and a 12v transformer with a 52 watt core. I'll try and get a picture of the bridge, and its leads up.

It is hard to be sure, but it might be the brushless motor Hall effect commutator driver IC. There should be about 3 of those in the fan.
